i am trying to save myself some time when i am in manila to get this completed. i think i might miss my flight to where i’m going after i leave manila. if you know where i can download it from or if you have a blank copy can u send it to me. i already looked at the embassy in manilas website and they offer no download forms. and also i get no responses to my emails to them either. thanks
i know i have to be there in person i just want the form so i can fill it out early to save time

AS FAR AS I KNOW, YOU HAVE TO GO PERSONALLY TO THE US EMBASSY IN MANILA FOR THAT FORM. YOU SIGN THE FORM & GET THE AFFIDAVIT THE SAME DAY.
