I am being offered a percentage of total revenue of a website that will be launched soon. I am providing much or all of the video content to this site…
Where can I find a template or fill-in-the-blank contract for this type of thing?

http://www.uslegalforms.com/ or http://www.allbusiness.com/ should help some. It would be wise though to have an attorney review the agreement though to make sure you are protected.
