and the couple has it notarized, is it a legally binding contract? Does it means the two persons can go on and live separate lives?

If what the template says is valid and legally binding in your state than yes.
A template is nothing more than an outline to follow. You take what you need and discard and/or add the rest.
There is nothing illegal about it.
