Where do I find the “contract number” for my OB Pre-Registration Form?

Question by Jillian: Where do I find the “contract number” for my OB Pre-Registration Form?
I’m 9 months pregnant, and I’m trying to fill out my Pre-Registration form for the hospital. My Primary insurance carrier is Amerigroup, but it says to put down contact number and group number, policy holder’s name, insurance phone number, employer, and employer’s phone number. Where are those numbers on the Amerigroup card?
