Or does the form have to be made up by an attorney or someone “official”? My husband plans on starting a lawn care business and wants customers to sign a contract, but we didn’t know if that is a legal document or if I could come up with one on Word.

yes, you can make up any contract between you and the customer that fits yours and the customer’s needs
you need to specify what is expected of each, price, dates, terms of termination etc. and of course, liability
when you and the customer both sign and each has a copy you have a legal document
