A contract for music being in a show?

I am working on a tv show and we have many bands playing in the show. I know many show have music playing their episodes. This is what I hand in mind:
Musician episode contract
We have the right to use “bands” song as background music. Band will be in episode credits, Cellar Door website and Babel Records website. From the moment the song is used in episode musicians lose all rights to their song and distribution of that song in the context of the episode. We have the right to edit or place music anywhere in episode. Musicians will not be getting paid but will be advertised as listed above.

Is there any contract out there that covers all that?
BTW all of the bands have signed onto this contract

None that any band or their representatives would ever sign.
